Abstract
Endometrial stromal tumors (ESTs) are rare tumors of endometrial stromal origin, usually affecting women of reproductive and postmenopausal age. In the 2003 World Health Organization classification, ESTs are divided into three categories: endometrial stromal nodules (ESNs); endometrial stromal sarcoma (ESS); and undifferentiated endometrial sarcoma (UES). This chapter addresses the ESS and UES tumors. The histologic differentiation between the two is based on tumor differentiation and similarity with endometrial stroma in the former. ESSs are low-grade malignant tumors, exhibiting tongue-like myometrial and lymphovascular invasion. UESs, in contrast, are highly aggressive malignancies with marked cytologic atypia, high mitotic activity, and little to no endometrial stromal differentiation. The correct classification and pathologic diagnosis of ESTs is of great clinical relevance since prognosis and treatment vary depending on tumor type.
