Abstract
An 80-year-old woman presented with a complete left third nerve palsy (TNP). Pupils measured 5 mm bilaterally and both reacted normally to light. She has been treated for hypertensive vascular disease for the past 25 years. The patient felt well, although she did report increasing headaches recently and some decrease in her overall level of energy.
