Abstract
A man aged 78 years fell out of a boat, hitting his head. He was resuscitated and hospitalized. On the 1st day in hospital, the general condition was satisfactory but the next day he became confused and somnolent. Mild dysphasia and weakness of the right facial nerve developed. Pupils and peripheral reflexes were symmetrical. Chest X ray revealed right sided aspiration pneumonia. The cranial X ray showed a 10 mm displacement of the calcified pineal body to the right. This finding, combined with the anamnesis and clinical findings strongly suggested acute subdural hematoma. However, a CT scan revealed a normal median position of the pineal body, and no signs of extracerebral hematoma. Only atrophic alterations were observed. The transverse diameter of the pineal body in the CT scan was 14 mm. The right hand side of the calcification amounted to 90, that on the left to 70 Hounsfield units.
