Combined phacoemulsificaton and trabeculectomy with mitomycin-C; SA vie versus different sites

Abstract

Purpose, To compare the efficacy of combined phacoemulsification with posterior chamber intraocular lens implantation and trabecukctomy with mitomycin -C performed from same versus different sites. Method. The data Vom 22 consecutive patients (25 eyes) who had superior trabeculectomy and temporal cle ir cornea phacoemulsilication surgery (group 1) and 9 consecutive patients (11 eyes) who had combined phacoemulsification and trabeculectomy through superior scierai tur nel (group 2) were evaluated retrospectively. The median follow-up period uas 18 months In group 1, 12 patients (12 eyes) had primary open angle glaucoma iPOAG). 10 (12 ey.-s) had combined mechanism glaucoma (CMG). In group 2, 6 (6 eyes) had POAG and 3 (i eyes) had CMG- The variables evaluated were pre and postoperative visual acuities, 1C P reduction, number of medications used and induced astigmatism The statistical me hods used were chi-squarc contingency table analysis and students t test. Results. The t est corrected visual acuity was better than 20/40 in 11 (44%) eyes in group 1 and 3 (27°/j) eyes in group 2. The mean preoperative and postoperative lOP's of group 1 were 21.(±4.6 and 13.8.±4.5 mmHg respectively. That of group 2 were 21.3±6.2 and 13.8±6.9 mmHg respectively. The number of medications used before and after surgery in group I were 2.1±.97 and 0.7±1.2 respectively. That of group 2 were 2.6±1.2 and 1.1±1.7 respectively. The visual acuities, IOP control and number of medications used in both groups postopera ively were comparable between two groups. The astigmatism induced by the surgery was 0.4±1.0 diopters in group 1 and 2.2±1.3 in group 2.(p<0.05). Conclusign. Clear corneal ph; coemulsification combined with trabeculectomy and mitomycin-C induces less astigmatism compared to (he same site combined procedure but both procedures produce comparab.e visual acuities, IOP control and need for medications after surgery.
