Abstract

Malignant gastric outlet obstruction (mGOO) is a late complication of pancreatic malignancy and presents a therapeutic challenge for the gastroenterologist and oncologist. Traditionally mGOO was treated with surgical gastroenterostomy. Serial advancements in endoscopy have provided minimally invasive alternatives including enteral stent placement and endoscopic ultrasound (EUS)-guided gastroenterostomy (EUS-GE). This chapter reviews the various therapeutic modalities available in the management of mGOO and provides a management algorithm based on a combination of currently available scientific evidence and expert opinion. Ultimately, the management of mGOO is multidisciplinary in nature and personalized based on the patient's immediate clinical situation and long-term prognosis.
