Two-Stage Hepatectomy for Bilateral Colorectal Liver Metastases: Experience of MD Anderson Cancer Center

Abstract

Historically, most patients presenting with bilateral colorectal liver metastases were determined to be unresectable. However, with perioperative advancements over time and novel technical approaches to liver resection, the 5-year overall survival rates for this group now approach 50%. The two-stage hepatectomy approach entails initial tumor debulking followed by time for interval growth of the future liver remnant and then final curative resection. This chapter will discuss the approach to two-stage hepatectomy at MD Anderson Cancer Center, including a recently described fast-track option. We will explore outcomes after two-stage hepatectomy and future areas for investigation.
